Welcome to Our Human Anatomical Donation Program
At Oregon Body Gift, our goal is to facilitate body donation for medical research and provide cremation and transport at no cost for donors. The registration form can be completed either by the donor or by their authorized agent, if death has already occurred. Thank you for your interest in contributing to science.
The registration process is simple and can be completed online in less than 10 minutes. If you are short on time, you can complete part of the form and then the rest of it later. If you have any questions, we are here to help, please call us anytime at 503-581-1942.
Informed Consent for Body Donation
Please note that the first form, Informed Consent for Body Donation to Oregon Body Gift, is required for all donors. It explains the process and allows for authorization of whole body donation. You can either fill it out online or download it and send it in. The remaining forms are optional in many cases.
Submitting Downloaded Forms
If you downloaded the forms, then once they are completed, you can submit them by email or regular mail:
- Save the completed forms with an electronic signature.
- Or print, fill out, and scan or take photos of the forms.
- Then, attach the forms to an email.
- Send to: donation@oregonbodygift.org
- Subject: Donation Forms
